【Pathogenic】Secret Boss — How to Reach the Brain

Brain unlock conditions

  • Complete a normal Heart ending first. The ending reveals the fixed code used on symbol walls: →, ↑, ↓, ↑, →, ← (Right, Up, Down, Up, Right, Left).

Step 1 — Find the hidden room and the tracker

The tracker appears in an early-level hidden room (from the Skin to the Intestine).

  • Hidden rooms (similar to Isaac secret rooms) never spawn in the starting room, boss room, item room, or shop room.

  • Attack suspicious walls to reveal one — look for a wall spot where two platelets are stacked together, usually near the map edge.

  • Inside, find the glyph and stand close to it. Enter the fixed code (→, ↑, ↓, ↑, →, ←) with the arrow keys on the keyboard (WASD does not work); use the D-pad on a controller.
  • A pulse wave radiates from your body (costs 1 HP).
  • Enter the room to collect the tracker (chemo-receptor antenna). When overcharged, it points toward the current organ’s secret location.
  • Connect the tracker to a long-duration Mitochondrionto keep it locating for longer.

Step 2 — Collect the four sigils

In the following levels, find the glyph rooms. Enter and collect the sigil.

Record each sigil’s shape and direction — take a screenshot.

  1. Intestine (Level 2) — the special blood-vessel room right after its boss. It spawns at a fixed spot and only opens if you beat the boss without losing HP (losing shield is fine). Costs 2 max HP.
  2. Stomach (Level 3)no fixed position; follow the tracker’s signal.
  3. Liver (Level 4) — the hidden room is fixed inside the Collector shop. Costs 40 cores.
  4. Lungs (Level 5)no fixed position; follow the powered tracker.

Step 3 — Final post-Heart input

Beat the Heart (the final level), then go to the symbol wall above the heart.

  • Read the sigil shapes, and use the four shape-and-direction pairs you collected to embed the sigils into the wall by pressing the arrow keys.
  • This is not the fixed code. Entering a wrong direction costs HP.

After defeating the Brain, you can enable “Skip brain puzzle” in the Difficulty settings.
